The Lhaid Ogo map is a very unique instance in that the maze itself is different for each character that runs it. I have 9 alts that I ran through this and sometimes different parts of the ceiling has collapsed forcing you to go a different way. So the path you outline may not necessarily work for every player. Thank you for the map- all of my characters missed #3. Maybe I can go back and do it, but I have a feeling I'm out of luck.
9 alts, yikes!
I did notice the map issue and was trying to figure out what I did wrong. Thanks for the clarification.
BTW, at least on Bullroarer, you could reenter the instance after you turned in the quest.
Just a small clarification - the Second Age LI you get after completing Chapter 4 is actually always a class/secondary weapon item.
